The following must be completed EVERY TIME a:

- student attends class at Bravo Dance Co. 

- an adult enters a studio with a student at Bravo Dance Co.

- a child or adult attends Bravo Dance Co. for any type of rental or function

By completing the form below you agree that you or your child:

• does not have a fever above 37.8 degrees

• is not displaying and has not displayed in the last 48 hours, any of the following symptoms: cough, shortness of breathe, nausea or diarrhea, sore throat and difficulty swallowing, new smell or taste disorder, or all cold or flu symptoms.

• has not been outside of Canada or had close contact with anyone who has been outside of Canada within the last 14 days.

• has not been in contact with someone who is confirmed or presumed to have COVID-19.

• Should me or my child become a COVID-19 carrier due to presumed contact with a carrier also present at Bravo Dance Co., I do not hold Bravo Dance Co., it's contractors, owners, or fellow clients responsible in any capacity.
